Please fill out and print a copy of the forms to be completed at the Town Office. Exclude your Social Security Numbers and DO NOT SIGN the document until you are in the Town Office. Read all of the instructions on page 3 before filling out the document.
Please call ahead to let us know that you are coming in and plan for approximately 40 minutes to complete the application process. Bring in a license or picture ID and the most recent divorce decree, with a raised seal, if you have been previously married.
"INSTRUCTIONS FOR PARTIES: Complete every item carefully. Type or neatly print in BLACK ink only. Check the boxes and sign the certification portion in the presence of a notary public, municipal clerk or the State Registrar. Return the completed intentions to the municipality in which at least one party resides or the State Registrar of Vital Statistics. If neither applicant is a Maine resident, parties may file the completed intentions in any municipality or the State Registrar of Vital Statistics."
